If you believe that JSH has failed to provide these services or discriminated in another way on the basis of race, color, national origin, age, disability or sex, you can file a grievance with our Patient Advocate at 270-887-6849, 320 W. 18th Street, PO Box 2400, Hopkinsville, KY, 42241.  You can file a grievance in person, by phone, or by mail.  If you need help filing a grievance, our Patient Advocate is available to help you.
